Role of Ilioinguinal Neurectomy in Lichtenstein Tension-Free Mesh Hernioplasty: An Observational Study (Code-T0136)

Introduction: Lichtenstein tension-free mesh hernioplasty is a standard surgical technique for inguinal hernia repair. Despite its effectiveness, chronic postoperative groin pain is a frequent concern, often attributed to ilioinguinal nerve irritation or entrapment. Ilioinguinal neurectomy has been proposed as a strategy to address this issue and enhance patient outcomes.
Aims and Objectives:
To study role of ilioinguinal neurectomy in Lichtenstein tension free mesh hernioplasty.
Methods: A prospective observational study was conducted on patients undergoing inguinal hernia repair with Lichtenstein tension-free mesh hernioplasty. Patients were divided into two groups: Group A received the standard procedure, while Group B underwent Lichtenstein hernioplasty with ilioinguinal neurectomy. Postoperative pain (using a visual analog scale), recurrence rates, and complications were monitored at intervals over a six-month follow-up period.
Results: Patients in Group B reported significantly lower rates of chronic groin pain compared to Group A, particularly at three and six months postoperatively. Recurrence rates were similar in both groups, and complications, such as transient sensory loss, were minimal and well-tolerated among patients in the neurectomy group.
Conclusion: Ilioinguinal neurectomy during Lichtenstein tension-free hernioplasty effectively reduces chronic postoperative pain without increasing complications or recurrence rates. This technique could enhance the quality of life for patients undergoing inguinal hernia repair. Further research is needed to confirm these findings in larger cohorts.
