Synopsis

Introduction: Medial knee osteoarthritis (OA) is a common cause of knee pain and disability, often leading to progressive joint degeneration. Proximal fibular osteotomy (PFO) has been proposed as an adjunctive procedure to alleviate symptoms by offloading the medial compartment. While its benefits are evident in certain patient populations, the short-term outcomes remain understudied.

Aims and Objectives:

Aim:

Clinical and Radiological outcomes of proximal fibular osteotomy (PFO) as a treatment for primary medial joint osteoarthritis (OA) of the knee.

Objective:

To describe the functional and radiological outcomes at one year in patients undergoing PFO for medial joint OA of the knee.

Methods: A total of 40 patients with symptomatic medial knee OA were enrolled in the study and underwent proximal fibular osteotomy. Preoperative and postoperative evaluations, including the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, visual analog scale (VAS) for pain, and knee range of motion, were performed at 6 months follow-up. Radiological assessments included the measurement of tibiofemoral angle and joint space width.

Results: At the 6-month follow-up, significant improvements were noted in pain (VAS), function (WOMAC score), and range of motion compared to baseline (p < 0.05). Radiological analysis showed a reduction in the varus deformity and an increase in joint space width in most patients. No major complications were observed, and patients reported high satisfaction levels.

Conclusion: Proximal fibular osteotomy provides significant short-term benefits for patients with medial knee osteoarthritis, improving pain, function, and alignment without major complications. Further long-term studies are needed to assess its durability and efficacy.
