Synopsis

Introduction: Liver cirrhosis, characterized by diffuse hepatic fibrosis and nodular regeneration, progresses from a compensated to decompensated state, manifesting complications like ascites and encephalopathy. Thyroid hormones (T3 and T4), which are synthesized and metabolized in part by the liver, exhibit alterations in cirrhotic conditions. This study explores the correlation between thyroid function and liver cirrhosis severity, assessed by the Child-Turcotte-Pugh (CTP) score.

Aims and Objectives:

Aim: To study the pattern of thyroid function test in liver cirrhosis patients and its association with disease severity.

Objectives:

  1. To determine thyroid function test in cirrhosis patients.
  2. To determine CTP score in cirrhosis patients.
  3. To correlate between serum free T3, free T4, TSH levels and CTP score in severity of cirrhosis.

Methods: A hospital-based cross-sectional study was conducted at Sri Siddhartha Medical College, Tumakuru, over 18 months. A purposive sample of 110 patients with clinically and sonologically confirmed cirrhosis was included. Data collection involved detailed history, clinical examination, and laboratory investigations, including thyroid function tests (free T3, T4, TSH) and liver function tests. CTP scores categorized patients into classes A, B, and C. Data were analyzed using SPSS, with statistical significance set at p<0.05.

Results: Preliminary findings revealed significant correlations between reduced free T3 levels and increased CTP scores, indicating worsening liver function. Free T4 and TSH levels also demonstrated notable trends correlating with cirrhosis severity.

Conclusion: Thyroid dysfunction, particularly reduced free T3 levels, is strongly associated with advanced liver cirrhosis and higher CTP scores. Thyroid function tests may serve as valuable prognostic indicators, aiding in disease stratification and management.
