Synopsis

Introduction: Synchronous double primary malignancies are uncommon, and their coexistence with intestinal tuberculosis is exceedingly rare. Such cases present unique diagnostic and therapeutic challenges, often requiring a multidisciplinary approach. This report highlights the clinical, diagnostic, and therapeutic aspects of a rare case of synchronous colon and right kidney malignancies associated with intestinal tuberculosis.

Aims and Objectives: The study aims to describe the clinical presentation, diagnostic approach, and management of synchronous primary malignancies of the colon and kidney with concurrent intestinal tuberculosis and to emphasize the importance of early recognition of such rare combinations.

Methods: A detailed case analysis was performed on a patient presenting with weight loss, abdominal pain, and altered bowel habits. Diagnostic modalities included contrast-enhanced CT, colonoscopy, and histopathological examination of biopsy samples. The clinical and pathological findings were correlated to establish the diagnosis and plan appropriate management.

Results: Imaging and histopathology confirmed synchronous adenocarcinoma of the colon and clear cell carcinoma of the right kidney. Additionally, intestinal tuberculosis was identified through biopsy and microbiological testing. The patient underwent a staged surgical intervention, including right hemicolectomy and nephrectomy, followed by antitubercular therapy. Postoperative recovery was uneventful, and follow-up revealed no recurrence.

Conclusion: The coexistence of synchronous malignancies and intestinal tuberculosis is an extremely rare clinical entity. Early diagnosis through advanced imaging and histopathology is critical for timely management. This case underscores the importance of a multidisciplinary approach in managing complex cases to optimize outcomes.
