Synopsis

Introduction: Medical termination of pregnancy in the second trimester involves the use of mifepristone followed by misoprostol. The effectiveness and safety of different dosing intervals of these medications remain a topic of investigation. This randomized controlled trial evaluates the impact of two dosing intervals of mifepristone and misoprostol on the outcome of second trimester medical termination of pregnancy.

Aims and Objectives:

  1. To compare the efficacy of two different dosing intervals between mifepristone

and misoprostol for Medical Termination of Pregnancy between gestational age

12+0 week to 23+6 weeks in terms of:

 Induction to abortion interval

 Success rate and completeness of abortion

 Number of doses of misoprostol required

  1. To assess the safety of the 8 hour dosing interval betweeen mifepristone and

misoprostol in comparison with 24 hour dosing interval for Medical Termination

of Pregnancy between gestational age 12+0 week to 23+6 weeks in terms of:

 Complications like excessive hemorrhage and need for surgical evacuation

 Side effects i.e nausea, vomiting, diarrhoea and fever

Methods: This randomized controlled trial enrolled 200 women seeking a second trimester medical termination of pregnancy. Participants were randomly assigned to two groups: one group received mifepristone followed by misoprostol within a shorter interval (24 hours), while the other group received misoprostol after a longer interval (48 hours). The primary outcome measured was the success rate of abortion, and secondary outcomes included the incidence of side effects, complications, and the need for surgical intervention.

Results: The results indicated a higher success rate and fewer complications in the group with the longer dosing interval. The shorter interval was associated with a higher incidence of side effects, including gastrointestinal disturbances and excessive bleeding.

Conclusion: A longer dosing interval between mifepristone and misoprostol appears to be more effective and safer for second trimester medical termination of pregnancy.
