Synopsis

Introduction: Lichtenstein hernia repair is a widely used technique for inguinal hernia repair. The choice of anesthesia—spinal or local—can significantly impact postoperative recovery and quality of life (QoL). This study evaluates and compares the QoL outcomes in patients undergoing Lichtenstein hernia repair under spinal versus local anesthesia.

Aims & Objectives

To find out better patient tolerated anesthesia in open mesh repair for inguinal hernia.

Methods: This prospective observational study was conducted on 205 adult male patients undergoing Lichtenstein hernia repair for unilateral inguinal hernia. Patients were divided into two groups based on anesthesia type: spinal (n=100) and local (n=100). Data were collected on intraoperative and postoperative pain (visual analog scale), hospital stay duration, postoperative complications, and QoL (measured using the SF-38 questionnaire) at 1 week, 1 month, and 3 months post-surgery.

Results: Patients receiving local anesthesia reported significantly lower postoperative pain scores (p<0.05) and earlier ambulation compared to those under spinal anesthesia. Hospital stays were shorter in the local anesthesia group (mean: 1.2 days vs. 2.5 days, p<0.05). QoL scores at 1 week and 1 month were higher in the local anesthesia group, though differences diminished by 3 months. Complication rates were comparable between groups.

Conclusion: Lichtenstein hernia repair under local anesthesia offers superior early postoperative QoL, reduced pain, and shorter recovery times compared to spinal anesthesia. These findings suggest local anesthesia as a preferred option for this procedure.
