Introduction: Postoperative pain management is crucial for below-knee surgeries to ensure better patient outcomes and recovery. Various regional anesthesia techniques are employed to achieve effective analgesia, including the Modified 4-in-1 Block, IPACK Block, and Adductor Canal Block (ACB). This study compares the efficacy of the Modified 4-in-1 Block and IPACK Block combined with ACB in providing postoperative analgesia following below-knee surgeries.

To compare the efficacy of modified 4 in 1 block Vs IPACK and ACB for Intraoperative and Postoperative analgesia.
Objectives :
1) To assess the duration of postoperative analgesia between groups.
2) To assess the ease of giving blocks.
3) To assess the post operative complications
Methods: A prospective, randomized study was conducted on 100 patients undergoing below-knee surgery. Participants were randomly assigned to receive either a Modified 4-in-1 Block or an IPACK Block along with ACB. Postoperative pain was assessed using the Visual Analog Scale (VAS) at regular intervals, and opioid consumption was recorded. The duration of analgesia and any complications were noted.
Results: The Modified 4-in-1 Block group showed significantly better pain control in the first 24 hours, with lower opioid consumption. The IPACK Block with ACB demonstrated longer-lasting analgesia and fewer side effects. Both techniques were effective, but the Modified 4-in-1 Block provided superior initial pain relief.
Conclusion: Both Modified 4-in-1 Block and IPACK Block with ACB are effective for postoperative analgesia in below-knee surgeries. The Modified 4-in-1 Block offers rapid and effective pain control, while the IPACK Block with ACB provides longer-lasting analgesia with fewer complications.
